Original Form 138 or Report Card duly signed by the Principal.

Original Certificate of Good Moral Character.

Clear copy of Certificate of Completion.

Original NCAE Result.

F.

Original Form 137 or Permanent Record.

G.

Four recent 2x2 ID pictures with white background.

Incoming Grade 12 Students

A.

Duly accomplished clearance form.

B.

Original Form 138 or Report Card duly signed by the Principal.

C.

School ID from the previous academic year.

Admission to Tomas Dizon Foundation Institute is open admission, and has the right to refuse acceptance or re-admission to any students whose behavior or academic standing is at variance with the vision/mission of the institution as a whole.
